Shipping Letter of Authority
Whereas:
1. The undersigned (hereafter referred to as the *Customer*) is the owner of the
credit card referred to below.
2. The Customer wishes to purchase certain merchandise from The Edge-Man
(hereafter referred to as the *Supplier*) from time to time and to charge such purchase
to his ___________(name of card) charge card, details of which are given below.
3. The Customer has specifically requested the Supplier to ship the merchandise
purchased to an address other that the address at which the Customer receives
his/her credit card bill (hereafter referred to as *the shipping address*).
4. The Supplier has agreed to ship the merchandise to the shipping address
provided that the customer authorizes it to do so in writing and completes all
the information required by this Letter of Authority.
5. By his/her signature to this Letter of Authority, the Customer irrevocably
authorizes the Supplier to charge purchases to the ____________(name of card) charge
card referred to below and to ship to the shipping address.
6. This Letter of Authority shall continue indefinitely until revoked by Customer
in writing.
